It's said "death" is the Doctor's constant companion. He goes places and people die (not necessarily through his direct action or inaction).

The gal in the background is a Death figure, as in, Neil Gaiman's Death, from Sandman.

Did you know that Death actually turned up in one of the novels (Happy Endings) where she told the Doctor off for his presumption for speaking for her "sister" Time?
